      3,000 Life Insurance Plan

1. What is the 3,000 Life Insurance Plan?

3,000 life insurance plan is a program offered by Cellcard in cooperation with Cambodia Life Insurance PLC. “Camlife” to all eligible subscribers of Cellcard. It provides a financial protection to life assured’s family in case of life assured risks to life due to all kinds of causes.

2. Features of the plan 

- Coverage period : 1 year;
- Payment period
: 1 year;
- Payment mode : USD 0.05 per day or USD 1.50 per month;
- Issued age : 18 to 50 years;
- Sum assured  
: 3,000 USD.

3. Benefits of the plan    

- Pay up to 100% of sum assured (based on the Percentage of Premium Collected (PPC)).

4. Do I need to register for 3,000 Life Insurance? If yes, how?

Yes, you do. Call Camlife’s Call Center at 061 431 111 (office hours).

5. General Exclusions

Camlife shall not pay the claim if the death of the life assured is due to:

1.    Suicide or defined murder;

2.    HIV infection, AIDS or AIDS related diseases;

3.    A committed/attempted criminal offense;

4.    Illness during the first 3 months from policy effective date;

5.    Pre-existing conditions during the first 3 months from policy effective date.

6. How to Claim?

Terms and Conditions

a) Registration for insurance means subscribers agree to submit their profile to Camlife.

b) One subscriber must get only one insurance policy (coverage).

c) Your free insurance package of $US300 will be ceased once you upgrade to this 3,000 life insurance.

d) If a subscriber registers multiple SIM cards for insurance, only a single policy is valid.

e) In case, a subscriber gets insurance more than one, when claim happens, Camlife only pays one.

f) Age of life assured must be from 18 to 50 (last birthday).

g) We only pay the valid claim to whom are registered for insurance with correct name to his/her phone number. For instance, a person registers for insurance under the name of other persons (your SIM card is registered under other’s person name). This case, we shall not pay the claim. On the other hand, if they first inform Camlife for correction, Camlife shall pay the claim when claim happens.

h) Premium deductions will begin after registration and cover will begin the next day at 00h:00s.

i) Claim amount shall be paid based on the Percentage of Premium Collected (PPC). PPC means the sum of Premiums Collected each Month (PCM) in percentage from the start of policy year divided by number of months within that year.

j) If PPC is collected less than 25%, claim shall NOT be paid.

k) If PPC is collected at least 25%, claim shall be paid in proportion to PPC. For example, if the premium collected is 75%, we will pay claim 75% (75% x 3,000 = 2,250) minus 10$ of admin fee. So the final claim amount is 2,250 – 10 = 2,240$.

l) Claims must be informed within 3 months of death of the insured, otherwise it is rejected.



 1. Who is eligible to this 3,000 life insurance?

Any Cellcard’s subscriber whose age is from 18 to 50 years of age (last birthday).

2. Do I need to pay for this 3,000 life insurance?

Yes you do. It is just 200Riels ($US0.05) per day or 6,000Riels ($US1.5) per month.

3. When can I know the 3,000 life insurance starting to cover me after my registration?

You will receive an SMS to confirm.

4. Why might I be rejected for the attempt of registration to this 3,000 insurance?

-  You phone number might not register under your name;

-  Your age might not from 18 to 50 years old;

-  Or your balance is not enough.

5. Can I apply for this 3,000 life insurance if my phone number (SIM card) is not registered under my name?

Yes you can. You can call Camlife at 061 431 111 (office hours).

6. How do I de-activate this 3,000 insurance from my phone number?

7. What are the required documents to submit for filing a claim to Camlife?

Required documents are:

  • Death claim form filled in ​by​ assured’s family (beneficiary*);
  • A copy of ID card/Passport/Birth Certificate of the assured. Original document is needed to verify;
  • ID Card/passport /birth certificate of beneficiary. Original document is needed to verify;
  • Death Certificate from competent authority. Original document is needed to verify;

8. How long does Camlife take to decide a claim after receive a complete set of document?

A claim shall be decided to in maximum of 7 working days after receiving the

complete required document.

9. How is claim amount paid to life assured’s family?

The amount can be paid through Wing transfer, bank transfer or cash (life assured’s family needs need to receive cash at Camlife’s office).
